La III Consensus Conference per il Mesotelioma Pleurico Maligno:stato dell'arte e raccomandazioni

INTRODUZIONE

Ecco un nuovo appuntamento relativo all'informazione scientifica ed alla divulgazione delle novità nell'ambito del mesotelioma pleurico maligno (MPM).
In questa occasione riteniamo utile rendere partecipi i lettori di una nuova consensus conference, ossia una riunione di esperti che ha portato alla stesura di un aggiornamento dello stato dell'arte di questo argomento ed alla raccolta delle raccomandazioni per la gestione di questa malattia.
Si tratta della III consensus conference italiana, che ha avuto luogo a Bari nel Gennaio 2015, sostenuta dall'AIOM (l'Associazione Italiana di Oncologia Medica).

Questo articolo è suddiviso in nove differenti capitoli: Introduzione, Metodi, Epidemiologia, Diagnosi, Accertamenti radiologici, Chirurgia, Radioterapia, Chemioterapia, Aspetti psicosociali e legali e prospettive future.
In questa revisione, verranno raccolte le referenze principali e verranno riassunti in modo schematico i concetti principali di questa Consensus Conference. Per maggiori approfondimenti e dettagli più specifici, si rimanda al testo completo.

Stato dell'arte e raccomandazioni per il Mesotelioma Pleurico Maligno, secondo gli esperti Italiani

Dati Epidemiologici

L'incidenza del MPM in Italia nel 2011 era di 3.49 e 1.25 casi ogni 100.000 persone/anno rispettivamente per gli uomini e per le donne. Sono stati riportati 1428 casi tra cui 1035 uomini e 393 donne (Anon, 2015). L'incidenza nazionale e la mortalità sono attualmente in corso di stabilizzazione e sembra di sia un trend ad un plateau. Tuttavia, si ritiene che si verificherà un picco nel 2020-2025, soprattutto nei paesi industrializzati.
Come è noto l'esposizione all'amianto è strettamente correlata con l'incidenza di MPM ed esiste una vera e propria relazione dose-risposta (Prince, 2005; Mastrangelo, 2014).
L'esposizione cumulativa all'amianto è un indice che tiene conto della somma dell'esposizione ed è utilizzato in diversi ambiti di ricerca, tuttavia non tiene conto di dati importanti quali la durata o l'intensità dell'esposizione stessa (Thomas, 2013; Lubin, 2006; Vlandereen, 2013; Richardson, 2012).
Tuttavia, è possibile affermare che, per quanto riguarda l'esposizione cumulativa all'amianto, sembra che non vi sia una grande differenza rispetto alle Consensus Conference pubblicate precedentemente (Pinto, 2011; Pinto, 2013).
L'esposizione all'amianto può essere di tipo occupazionale, cioè legata al lavoro che il paziente esercita o ha esercitato in passato, oppure non occupazionale, legata soprattutto all'inquinamento atmosferico e domestico. In Italia si stima che l'esposizione non occupazionale avvenga in circa il 10.2% dei casi (Registro nazionale mesoteliomi, 2012).
L'asbesto può diffondersi per via aerea sotto forma di fibre. La WHO (World Health Organisation) ha stimato che un'esposizione continua a 0.4-1 fibra/l può causare un rischio di ammalarsi di MPM in 0.4-0.5 casi su 100.000 persone (World Health Organisation Regional Office for Europe, 2000).
Inoltre, l'asbesto può essere presente nell'acqua, tuttavia non ci sono evidenze di casi di mesotelioma secondari ad ingestione di fibre.
Sono stati documentati casi di talco contenenti asbesto, sebbene questo non si sia mai verificato nel territorio italiano (Finkelstein, 2012).
Altri cancerogeni legati all'amianto che sono stati causa di MPM in Italia sono la fluoro-edenite, come è accaduto nell'aerea di Biancavilla (CT). Questi casi sono stati simili a quelli descritti in Giappone nelle aree vulcaniche.
Sebbene vi siano dati di cancerogenesi solo nell'animale da esperimento e non siano mai stati descritti casi di MPM nell'uomo, anche il carburo di silice è un agente cancerogeno potenzialmente causante questa malattia (Grosse, 2014).
Esistono anche dei casi ereditari di MPM, legati ad alterazioni genetiche come per esempio le mutazioni di BAP1 (Klerk, 2013; Betti, 2015. Anche in Italia sono stati descritti alcuni di questi (Ascoli, 2007; Ascoli, 2014).

Diagnosi

Spesso il MPM si manifesta con un versamento pleurico; tuttavia, questa raccolta di fluido nella cavità pleurica può anche essere secondaria a differenti patologie. Pertanto, è importante prima di tutto procedere con una diagnosi differenziale tra il tumore primitivo della pleura, il MPM, e le altre neoplasie che possono metastatizzare a livello pleurico, tra queste le più comuni sono il polmone, la mammella ed il rene (Smith, 2014). Inoltre, è importante ricordare che entrano in diagnosi differenziale molte patologie non neoplastiche che possono tuttavia causare versamento pleurico: per esempio, pleuriti infettive o infiammatorie, insufficienza cardiaca, versamento parapneumonico...
La diagnosi di MPM viene effettuata soprattutto tramite un'analisi di una biopsia pleurica, che solitamente si ottiene tramite toracoscopia o più raramente per mezzo di una biopsia percutanea eco- o TC-guidata (Pinto, 2013; Scherpereel, 2010; van Zandwijk, 2013).
Oltre all'analisi istologica che viene effettuata sul tessuto ottenuto tramite biopsia, è possibile fare una analisi citologica, valutando le cellule presenti nel liquido pleurico. Questo può consentire in alcuni casi la diagnosi, ma è un esame che non è così sensibile come l'istologia (Kawai, 2014; Paintal, 2013; Hjerpe, 2015, Henderson, 2013).
In accordo con la WHO, è stata definita una classificazione istologica del MPM, che può essere suddiviso nell'istotipo epitelioide, quello sarcomatoso e quello bifasico (Larsen, 2013).
Differenti marcatori vengono utilizzati per definire meglio queste caratteristiche tumorali ed, in particolare, per differenziare le metastasi pleuriche da adenocarcinoma e le lesioni primitive da MPM (Ordonez, 2013; Betta, 2012).
I marcatori utilizzati più comunemente per differenziare il MPM epitelioide dagli adenocarcinomi sono: calretinina, D2-40 (anticorpo anti-podoplanina), la proteina del tumore di Wilms-1, le citocheratine 5 e 6, la mesotelina e la trombomodulina. Marcatori considerati negativi sono il CEA, BerEP4, MOC-31, claudina-4 e CD155 (Henderson, 2013b; Lonardi, 2011, Jo, 2014). La napsina A, TTF1, CDX2, PAX-8, marcatori apocrini e recettori ormonali sono invece utili per differenziare il MPM da altre metastasi localizzate a livello pleurico. BAP1 è un marcatore che è stato recentemente testato per differenziare le lesioni mesoteliali benigne da quelle maligne (Cigognetti, 2015).
Il MPM sarcomatoide esprime soprattotto marcatori quali pan-citocheratina, vimentina, marcatori di differenziazione muscolare liscio, D2-40, calretinina (Pinto, 2013; Ordonez, 2013; Scherpereel, 2010; Churg, 2015; Henderson, 2013b).
Altri marcatori utili per la diagnosi sono il peptide correlato alla mesotelina (SMRP), l'osteopontina, e la fibulina-3 (Lao, 2014; Creaney, 2011; Hollevoet 2011; Hollevoet, 2010; Luo, 2010; Wheatley-Price, 2010; Creaney 2014a; Franceschini, 2014).

Accertamenti radiologici

Le metodiche radiologiche principalmente utilizzate per il MPM sono differenti (Hallifax, 2015).
Il primo approccio avviene solitamente tramite una radiografia del torace, che solitamente permette di visualizzare la presenza di versamento pleurico o pericardico ed eventualmente lesioni peluriche molto estese.
La TC del torace è, invece, considerato un esame di seconda scelta, che consente di ottenere informazioni morfologiche molto più dettagliate rispetto alla radiografia del torace.
L'ecografia può essere utile per visualizzare alcune anomalie pleuriche specifiche, oltre che come guida per effettuare la toracentesi ed eventualmente per la guida delle biopsie pleuriche.
L'utilizzo della PET può essere applicato soprattutto per valutare il metabolismo di alcune lesioni, non si è modificato rispetto alla precedente consensus conference (Pinto, 2013).
Per quanto riguarda le procedure diagnostiche invasive, non vi sono modifiche nelle indicazioni e nelle raccomandazioni rispetto a quelle descritte dalla precedente revisione di esperti (Pinto, 2013).
La toracentesi rimane il primo approccio diagnostico minimamente invasivo e l'analisi citologica può essere utile per diagnosticare la presenza di cellule maligne in circa il 60% dei casi. La procedura di toracentesi applicata sotto guida eco può essere utile per ridurre al minimo le eventuali complicanze (Hooper, 2010).
La biopsia eco- o TC-guidata ha rimpiazzato definitivamente quella eseguita alla ceca ed è utile per biopsiare in modo preciso lesioni, anomalie o ispessimenti pleurici (Maskell, 2003; Qureshi, 2006; Adamset, 2001; Metintas, 2010a).
La toracoscopia è la tecnica diagnostica invasiva considerata il gold standard e che consente di ottenere la diagnosi nel 90% dei casi (Churg, 2014;Boutin, 1993; Hansen, 1998; Galbis, 2011; Brimset, 2012).
La tecnica di ultrasuoni endobronchiale (EBUS), utilizzata per analizzare i linfonodi che drenano le cellule neoplastiche derivanti dal MPM, può offrire alcuni vantaggi rispetto alla mediastinoscopia (Rice, 2009; Tournoy, 2008; Zielinski, 2010; Richards, 2010).
Tutte le tecniche radiologiche che vengono utilizzate per la diagnostica hanno un ruolo fondamentale anche nella stadiazione di malattia che consente, oltre alla determinazione della prognosi, anche la definizione degli approcci terapeutici che ovviamente variano a seconda dello stadio di malattia.
Le metodiche maggiormente utilizzate per questo scopo riamgno la TC e la PET (Truong, 2013a; Nickellet, 2014; Basu, 2011; Erasmus, 2005; Rice, 2009; Flores, 2003; Sørensen, 2008; Truong, 2013b; Armato, 2013; Frauenfelder, 2011; Labby., 2013a; Labby, 2013b; Byrne, 2004).

Approci terapeutici

Chirurgia

La chirurgia ha un ruolo nell'approccio diagnostico, in quanto tramite le metodiche invasive, anche precedentemente descritte, è estremamente utile per ottenere il materiale istologico (Greillier, 2007; Buenoet, 2004; Attanoos, 2008 ).
La chirurgia viene impiegata anche nel trattamento del versamento pleurico maligno. Infatti, la toracoscopia oltre che diagnostica può essere medica e consentire la somministrazione intrapleurica di talco al fine di ottenere una pleurodesi. Allo stesso modo è possibile applicare metodiche di drenaggio chirurgico specifiche per ogni caso clinico (Waller, 1995; Halstead 2005; Martin-Ucar, 2001; Nakas, 2008).
Naturalmente il ruolo della chirurgia del trattamento del MPM mira alla resezione completa della malattia. E' importante ricordare che questo è possibile solo nei casi di malattia resecabile e, quindi, è applicabile solo agli stadi più precoci (Rice, 2011 Aug; Gomez, 2014; Treasure, 2014; Flores Pass, 2008; Lang-Lazdunski, 2012; Taioli, 2015; (Cao, 2014; Sugarbaker, 2014; Nakas, 2012 ).

Radioterapia

Precedentemente la radioterapia veniva utilizzata per trattare il decorso del tratto utilizzato per l'accesso della toracoscopia o dei drenaggi pleurici. Si riteneva, infatti, che irradiare questo tratto diminuisse la possibilità di sviluppare metastasi lungo il corso anatomico dell'ottica o del drenaggio utilizzati nelle manovre invasive. Tuttavia gli studi sono discordanti e a tutt'oggi non esistono evidenze tali per cui questa indicazione radioterapica debba essere perseguita (Boutin, 1995; Bydder, 2004; O’Rourke, 2007).
Non ci sono dati randomizzati in supporto dell'utilità della terapia adiuvante per il MPM, tuttavia si ritiene che una dose totale di 54 Gy possa esser associata ad un ridotto fallimento del trattamento locale (Rusch, 2001). Differenti studi hanno confrontato la tecnica radioterapica ad intensità modulata con la radioterapia standard. Tuttavia la radioterapia efficace sembrerebbe quella applicata sull'intero emitorace coinvolto dalla malattia (Forster, 2003; Rice, 2007; Stahel, 2014). Attualmente sono disponibili alcuni dati preliminari sul potenziale utilizzo della radioterapia ad intensità modulata, utilizzata per risparmiare il polmone contenuto nell'emitorace affetto da MPM (Rosenzweig, 2012; Minatel, 2014; Chance, 2015).
La radioterpia palliativa è sicuramente fondamentale per il controllo dei sintomi ed in particolare per la gestione del dolore (Bissett, 1991; Lindén, 1996; MacLeod, 2015)).

Chemioterapia

Le indicazioni chemioterapiche standard sono state ampiamente descritte nella precedente Consensus Conference (Pinto, 2013).
Tuttavia, si ricorda che la prima linea terapeutica di questa malattia si basa sulla somministrazione di una combinazione a base si sali di platino e antifolati di terza generazione (Fennell, 2008; Muers, 2008; Vogelzang, 2003; Van Meerbeeck, 2005; Santoro, 2008; van den Bogaert, 2006; Buikhuisen, 2013, Anon, 2016; Ceresoli, 2008; Ceresoli 2014).
Le nuove conoscenze volte a comprendere le vie patogenetiche di questa malattia hanno consentito di identificare dei nuovi target terapeutici anche di tipo biologico (Kindler, 2012; Zalcman, 2010, Zalcman, 2015; Hassan, 2014).
La terapia di seconda linea non ha consentito evidenti miglioramenti della sopravvivenza rispetto alla sola terapia di supporto, sebbene alcuni farmaci standard, come il Pemetrexed, abbiano apportato un dati favorevoli in termini di miglior risposta obiettiva e controllo del tasso di malattia (Jassem, 2008; Ceresoli, 2014). Non ci sono agenti farmaceutici approvati per la seconda linea di trattamento e, pertanto, la possibilità di arruolare i pazienti in studi clinici potrebbe essere considerata come una buona opportunità di cura.
Non esistono ancora evidenze scientifiche confermate riguardo all'utilizzo di farmaci biologici in seconda linea (Ceresoli, 2014; Krug, 2015; Calabrò, 2013; Anon, 2016; Alley, 2015; Ceresoli, 2011; Bearz, 2012; Zucali, 2012).

Conclusioni

Sfortunatamente l'efficacia delle attuali terapie per il MPM è ancora molto limitata e la prognosi di questa malattia resta purtroppo infausta.
Nuovi approcci terapeutici sono necessari e la ricerca in questo ambito sta offrendo dei risultati interessanti, che necessitano di studi confermativi, randomizzati, multicentrici e riproducibili.
Nel frattempo, è utile proseguire con l'attenta sorveglianza dei soggetti a rischio e pertanto esistono una serie di consigli che possono essere facilmente applicabili.
Infatti, i programmi di sorveglianza che si stanno attuando mirano a diversi obiettivi quali:

  • informare i soggetti esposti all'amianto de possibili rischi ad esso collegati sia per l'esposizione presente che per quella passata;
  • informare i parenti dei soggetti esposti all'asbesto e la possibilità di rischio che anche per questi individui, sebbene non abbiano avuto un esposizione diretta;
  • ricostruire in maniera scrupolosa la storia occupazionale del paziente soprattutto entrando nei dettagli dell'esposizione ai cancerogeni, della sua durata e della sua intensità;
  • provvedere alla divulgazione delle informazioni riguardanti gli strumenti diagnostici, terapeutici e le prospettive mediche presenti anche all'estero;
  • supportare i reclami per ottenere compensi e risarcimenti;
  • effettuare un corretto counseling riguardante la cessazione del fumo di sigaretta ed il corretto stile di vita-salute.

Prospettive terapeutiche future sono vicine ed una serie di ricerche in corso offrono nuove speranze per il trattamento di questa patologia.
